* Must complete BEFORE October 15th

* If the DCHS Wellness Assessment is not completed before October 15th you will be charged and additional 5% of the total cost of your medical plan.

* Complete the DCHS Wellness Assessment at http://www.blueshieldca.com/hlr

* You will need your member ID# found on your insurance card.

* BOTH HMO AND POS PLAN PARTICIPANTS MUST COMPLETE THE DCHS WELLNESS ASSESSMENT.

* Upcoming: Wage Option Associates will need to provide proof of health insurance.
